If sending an email, please include your full name and a phone number where we can reach you between 8 a.m. and 5 p.m. MST. Within 24 hours, we'll contact you to schedule an appointment with one of our physicians.

Preauthoization for prescription medication

As prescription prices continue to increase, some insurance plans require pre-authorization of medications in order to decrease their costs. If your insurance company requires pre-authorization on medications, please call your plan's 1-800 number and request that a pre-authorization form be faxed to the Cheyenne Skin Clinic at (307) 635-1924. If your insurance company does not have a specific pre-authorization form, we will use our clinic form. Our nurses and physicians will complete the form and return it by fax to your insurance company. Our office will communicate with your insurance company by fax only, we will not call them.

It is possible that even with the proper pre-authorization documentation, your insurance company may refuse to pay for the prescribed medication. This does not mean that you do not need the medication. We prescribe medications that we believe are necessary and we would recommend that you fill your prescription if possible.

Ordering OTC Products:

To order any of our over-the-counter products, you may call, fax, or email us. If sending a fax, you may print out the order form on our products page and fax to (307) 635-1924.

Please include the product identification code, your full name and a phone number where we can reach you between 8 a.m. and 5 p.m. MST.
