Fees and Billing |

Our fees are based upon what is usual and customary for dermatology in the intermountain area. |

You are responsible for the total fee charged. Patients
sometimes have the understanding that their insurance should cover the
fee in full. Most, however, have a deductible which must be met first,
and may pay only a percentage of the fee. We will file with your insurance
carrier, however, your contract is between you and your insurance company. |

Medicare |
| |
Accepting Medicare Assignment means that we will accept what Medicare
will allow. Medicare will pay 80% of what they allow, and that is only
after all deductibles have been met. By law, we must collect everything
Medicare allows whether the 20% comes from you personally, or from a
secondary insurance carrier. |

If you are a Medicare patient, please complete the Medicare
Patient Information Form and
bring it with you to your first appointment. |

We will file your secondary insurance for you, if we are given full information to process your claim. While we are happy to provide this help, you are responsible for the total amount allowed. |
